Help for professionals

Weldmar Hospicecare Trust supports professional healthcarers from both within the Trust and in the wider community. We do this by delivering education and training in palliative care and offering advice on palliative care. We also advise health and social care professionals on making patient referrals to us.

Referrals

Access to our services for patients is by referral, normally from a patient's GP or consultant. The referral form could also be completed by the District Nurse or specialist home care nurse, or Social Services, in liaison with the patient's GP.
