The Dorset Clinical Commissioning Group (CCG) wants to hear your story and experiences
The CCG are inviting people to participate in their project gaining feedback and opinions on end of life care.
Weldmar Hospicecare is an independent charity and we need to raise over £18,000 every single day to continue providing our compassionate and individualised vital care completely free of charge to patients and their loved ones in Dorset. In addition to the money that we raise, from our chain of charity stores, fundraising events and activities, donations and gifts in Wills, we receive 21% of our overall income from the Dorset Clinical Commissioning Group (CCG).
The Dorset CCG is currently conducting a survey to help shape the future of palliative care in the community services across Dorset, and they want to hear from those who have had experience of end of life care over the last three years. Whether you are someone who is currently receiving end of life care, are a family member connected to a loved one receiving care, are a health care, community or voluntary sector professional – they want to hear from you.
Weldmar Hospicecare is proud to be able to tailor our care and support around each individual to respect their wishes and personal choices. The Dorset CCG understands the importance of identifying what matters to patients and families who have experience of end of life care as part of the planning process for this service.
