Weldmar Hospicecare is looking to appoint an Assistant Practitioner to support the Weldmar Community Nurses (WCNs) and North Dorset and allied health professionals in the delivery of high quality and palliative care to patients and their families predominantly in their own homes or Weldmar day services.
The post holder will work as a member of the multi-disciplinary team, participating in the assessment of care needs and the implementation of delegated care programmes to ensure delivery of the highest standard of palliative and end of life care to patients and their families.
Although the post holder will be predominantly working in the patient’s own home, the post holder will be expected to work flexibly across care settings when this is conducive to meeting the holistic needs of the patient and their family.
The post holder will be based in Sturminster Newton, covering North Dorset.
The post holder will have a Foundation degree in health and social care or equivalent qualification.
